As most Americans are aware, the Health Insurance Marketplace opened for public enrollment on October 1, 2013. Since then, many improvements have been made to ease the application process for the health insurance options.
There are four (4) main ways that a person can apply for Marketplace insurance coverage: online, by telephone, by paper application, or in person with an assistant. Although the Marketplace website (www.healthcare.gov) had start up problems, it is still the most real time method to shop for your own personal insurance and compare health plans. A person can create an account for the system, fill out the application, and obtain real-life, personal quotes for the various insurance plans and tax credits available.
To complete a telephone application, you can call 1-800-318-2596 where the Marketplace has customer service representatives to work with you to complete the application and enrollment process. Paper applications are also available by calling the number above. A PDF version of the paper application was added to the Marketplace website, so it can also be printed from there.
But here at Barnes-Kasson Hospital, we realize many people don’t feel comfortable using technology or have the technology available to use. We have trained staff members that are certified as application counselors to assist you in the application process. Our counselors will provide one on one assistance to complete your application or assist you with an ongoing application.

If you would like paper copies or a CD of your full Medical Records, please fill out the Authorization form below,
print, and bring it to our Medical Records department. The Authorization form may be faxed but needs to be accompanied by a photo id and signature match, such as a driver's license. The first copy of your records will be free but there is a charge for additional copies.
Fees are as follows: Pages 1-20 $1.89, over this records are not able to be faxed.
Pages 21- 60 $1.40
Pages 61-end $0.49

2Our Patient Portal provides information such as patient demographics, problems, medications, medication allergies, laboratory tests, laboratory values/results, vital signs, care plans including goals and instructions, procedures and care team members. In addition, encounter diagnoses, immunizations, referral reasons and discharge instructions may be available.

We realize that the clear exchange of information between patient and provider is essential to providing quality care. We provide all of our patients with qualified interpreters for languages other than English and for American Sign Language (ASL).
If at any time you need a professional non-English spoken or sign language interpreter, just ask and interpretation services will be provided.

Falls Can Happen
Barnes-Kasson is here to ensure your safety and security while you are hospitalized. If your physician allows you to walk, please use extra caution when getting in/out of bed, and when walking in the halls or to the bathroom. All bathrooms are equipped with an "emergency call button" to alert the nurses' station that you need assistance to do things that may cause a fall or strain.
Leaving the Floor
If you are planning to leave the floor, or visit another unit, please check first at the nurses' station to make sure your doctor has given approval for you to do so and to let the staff know where you can be located. It is important to stay in your room if a doctor, or other staff, is expected to be visiting you or if a treatment or distribution of medication is expected.
Electrical Appliances
All electrical equipment brought into the hospital must be checked by the Maintenance Department staff before being used. Examples of such items include cell phone chargers, hair dryers, electric razors, etc. Please have all electrical equipment checked before using it in the hospital; your nurse will be happy to make arrangements for the electrical safety check for you.
Identification Badge Policy
All members of the Barnes-Kasson Hospital staff and volunteers are required to wear an identification badge with his or her name and job title clearly printed on it. If you do not see a badge on someone involved with your care, please ask who they are, and to what position they are employed.
Fire Alarm
Fire drills are required every month and are done on all shifts. These drills are performed to keep our staff ready to perform should the need ever arise. If the condition is serious, you will be alerted immediately. You should be aware that when there is a fire alarm, a signal is tripped that closes, but does not lock, doors in certain areas.
Smoking Policy
Barnes-Kasson Hospital is a smoke-free facility. Smoking on hospital grounds is prohibited.
Medications
Patients are not permitted to bring in their own drugs or keep personal medications at their bedsides. If you have specialized medications that do not pertain to your present illness, please discuss your needs with the nurse that does your admission.
Meals
At Barnes-Kasson Hospital, nourishing and well-balanced meals are an important part of your treatment and recovery. Barnes-Kasson provides nutritious meals that are prepared according to your doctor's orders.
Your meal may be delayed if you are scheduled for a special test or treatment. You will be served after your examination/test as soon as your physician approves your meal service.
You will receive menus to select your meals for the next day. Please mark your menu so it will be ready for pickup. If you have difficulty making menu selections, ask for help. We will be glad to give you a hand.
If you are on a special diet prescribed by your doctor, you will receive menus designed for your needs. Restricted or special diets are closely monitored. If you are interested in guidelines for home use, the dietary staff will be happy to assist you.
Guest Trays
If a family member or friend wants to eat in your room with you, the person may go to the cafeteria, buy a meal there, and take it back to your room. Guests are always welcome to eat in the cafeteria.

Dinner: 4:00PM - 5:30PM
Vending Machines
Vending machines for snacks and beverages are located throughout the building for your convenience.
Television
Color television sets are available in each room; however, they are for the use of all patients in the room. Your consideration of other patients is appreciated.
Telephones
Telephones are provided in each room. Patients may receive calls in their rooms at any time. Local calls may be made at any time from the room by dialing 9 + the full 10 digit phone number (including area code). Long-distance toll calls can be made at any time by asking the nursing staff for assistance.
Your telephone extension is your patient room number
If you are in the ICU/CCU, your calls must go to the nurses' station at extension 276.
